Our goal is to respond to your AlternaVisit request as quickly as possible. For that reason, please be aware that your physician or one of their call partners may respond. Additionally, responses are typically delivered between 6:00am and 9:00pm. If you submit a request overnight, you may not receive a response until morning.

IMPORTANT INFORMATION REGARDING OUR PRACTICE

The information below is intended to provide you with details of the policies and procedures our practice follows.

ACCEPTING NEW PATIENTS

Dr. Cochran and Nurse Practitioner Smith are currently accepting new patients; Dr. Williams is not accepting new patients, with the exception of close family members of existing patients.

ALTERNAVISIT AFTER-HOURS REQUEST

It is our goal to respond to AlternaVisit care requests within 3 hours, between the hours of 6:00 AM and 9:00 PM. Overnight, between 9:00 PM and 6:00 AM, it is unlikely you will receive a response before the morning.

IN-OFFICE APPOINTMENT CANCELLATION

The policy of our practice is to charge a $25 fee for appointments missed and not cancelled at least 24 hours prior to the scheduled appointment time.

PRESCRIPTION REFILLS

We do not refill prescriptions for conditions that are not part of a long-term therapy. For certain prescription refills, we may ask you to schedule an appointment – as additional evaluation is sometimes necessary to ensure the appropriate continued treatment of your child.  All prescription refill requests should be called in a week ahead of time. Failure to give an adequate lead-time for refills may leave us unable to fulfill your request.

BILLING AND INSURANCE

For office visits, checkups, immunizations, and lab services, it is not our policy to bill for services rendered. We expect payment in full for each day’s services. We accept cash, checks, certified funds, Visa, MasterCard, Discover, and American Express. We automatically file insurance for companies that have direct payment contracts. With these plans, the customer is responsible for a co-pay or deductible amount at the time of service, and the balance is billed to the insurance company.

PATIENT INFORMATION

It is the responsibility of the patient, or the patient’s parent or guardian for children, to notify the practice of any changes in contact information, address, phone number, or insurance information. Failure to update this information can lead to the patient being responsible for payments for care received.
